Dr. Jeremy L Bradley was raised in Hartford, KY by his parents Pam and Jerry Bradley. He is a 1987 graduate of Ohio County High School and graduated from the University of Kentucky in 1991 and the University of Kentucky College of Medicine in 1995. He completed a three year Family Medicine residency program at Trover Clinic Foundation in Madisonville before settling in Owensboro with his family. He has been married for twenty eight years to his high school sweetheart, Kerri, and they have three children: Cora, Olivia, and Evan. Dr. Bradley is board certified in Family Medicine. He has obtained fellowship status with the American Academy of Family Physicians (FAAFP). Dr. Bradley serves as a gratis faculty member for both the University of Kentucky and University of Louisville Colleges of Medicine, teaching medical students. He also is an honorary staff physician at Vanderbilt University Hospital and is on staff at Owensboro Health Hospital. Dr. Bradley is a 5 star rated physician by Health Grades and his office has been recognized by Anthem, Humana, United Healthcare, and Community Care Networks' benchmarks for quality and efficiency by receiving the Bridges to Excellence Awards for Chronic Disease Management and Physician Office Systems. He also has provided volunteer services to both Daviess County High School and Owensboro Catholic School athletic departments as well as United Soccer Club. He is currently the company physician for Unilever Best Foods in Daviess County and Young's Manufacturing in Ohio County.
Dr. Bradley's Family Practice clinic is located in Owensboro, Kentucky, and opened its doors in April 2000. Owensboro is a federally-designated primary care physician shortage location. The clinic currently employs a board certified nurse practitioner, Colette Johnson. The office provides annual wellness exams for both children and adults along with chronic disease management, acute care services, radiology services, in house laboratory, EKGs, stress testing, PFTs, joint injections, DOT physicials, and multiple minor surgery procedures to more than 9,000 active patients. Being from this rural area, Dr. Bradley is committed to delivering the "Triple Aim" philosophy of providing high quality, accessible, and affordable healthcare services to his community. His office utilizes Electronic Health Records to better serve their patients and meet those goals.
Dr. Bradley participates in Aledade Accountable Care Organization (ACO) to help keep patients healthy.
